I've always thought I got a dud G3258 chip since I am only able to achieve a 4.1GHz overclock at around 1.320 Adaptive offset Core Voltage. It seems to crash less frequently with the CPU overclocked, but that's probably just my imagination, it crashes at default and overclocked settings.
The computer has crashed this way ever since I put it together, it does not appear to be worsening. Equally probable is a complete freeze where the game stays up, the computer becomes unresponsive, my fans speed up for a few seconds and then slow way down until I manually turn off the pc.

Often I get an "Nvidia Driver has stopped working" error ("application has been blocked from accessing hardware" on Windows 10). In a nutshell the computer runs high graphics fast and smooth under significant demand (instance boss fights) without stuttering or anything, but seemingly at random it will crash or freeze. I couldn't get a wireless driver to work so I wiped again and installed windows 8.1 which is working a little better than 10(driver stops and sometimes recovers on 8.1/doesn't recover ever on 10).
